I Built a Resource to Dismantle Transmedicalism
Here's Why.
About every ten years or so I find that this debate rears its ugly head. A little over a decade ago, some professional trans folks took up the challenge and deconstructed, debunked, and documented some of the debate and core issues. As things in the US and UK seem to spiral in a never ending loop of bad to worse for trans folks, it should come as no surprise that this is making a resurgence in topics and debates.
So today we are going to talk about Transmedicalism, Harry Benjamin Syndrome (HBS), and Transsexual Separatism (TS Separatists). What they are, why we should be aware or concerned about them, and what we can do.
To this end, I have built a resource that does something different than most arguments about transmedicalism. It does not start with accusations. It starts with what the research actually shows, then explains why people defend claims that the research disproves. And it does this systematically, across five chapters, using evidence instead of anger. This has been an on-going effort for some time, and I am glad to finally share this with you.
What you get:
- Chapter 1: The philosophy underneath transmedicalism (biological essentialism, respectability politics, gatekeeping)
- Chapter 2: The evidence—medical, legal, and strategic reasons the framework fails
- Chapter 3: Why people who survived gatekeeping often defend it, and what that costs, and why it persists.
- Chapter 4: How transmedicalism replicates oppressive systems before it, including its connection to white supremacy
- Chapter 5: Four independent scholars working in different disciplines who reached the same conclusion, and my converging analysis on them.
- Quick Reference: A table of refutations for actual arguments
Why This Matters
Transmedicalism is not a benign disagreement about definitions. It actively harms trans communities by:
- Restricting access to care based on arbitrary suffering requirements that disproportionately impact trans people of color, nonbinary people, and those without resources to perform "legitimacy"
- Fracturing political unity when some trans people distance themselves from others, making collective advocacy weaker against coordinated anti-trans legislation
- Replicating white supremacy by centering respectability and medical legibility as the path to acceptance—a path that has historically excluded and harmed the most marginalized trans people
The toolkit walks you through the evidence on every count, so you can understand not just what transmedicalism is, but how and why it causes harm.
Access the Resource
You can read the full resource on the wiki wiki.valah.blog or with this easy to remember URL: transmedicalism.info
Everything cross-references. You can start wherever you need to start. It's available as both a static site and a wiki mirror.
I'm not claiming this will change everyone's mind. Some people benefit from gatekeeping and won't give it up. But for everyone else; for the people genuinely questioning about what transmedicalism is and why it matters, or for those who are experiencing it for the first time and want to know more, now we have this.
I hope it helps.
